Voluntary Disclosure Agreement: The Bureau of Unclaimed Property encourages businesses to voluntarily comply with Pennsylvania’s Unclaimed Property Law by filing a report annually on April 15.  For businesses that have never filed a report and would like to come into compliance, the Bureau of Unclaimed Property offers a Voluntary Disclosure Agreement to bring companies up-to-date with their annual filing while receiving amnesty from penalties and interest.  Download form...

Businesses, organizations, and government entities in Pennsylvania must file an unclaimed property report with Treasury by April 15 of each year. Any holder that does not file a report by April 15 may be subject to interest and penalties.  These reporting requirements are supported by law under the Pennsylvania Abandoned and Unclaimed Property Act.

The Pennsylvania Treasury Department's Bureau of Unclaimed Property accepts unclaimed property reports in written format as well as by electronic means. To simplify the electronic reporting process and conform to national standards, the Bureau of Unclaimed Property has adopted the National Association of Unclaimed Property Administrators (NAUPA) reporting format. This format is accepted by more than 30 states. If you report property to more than one state, chances are you will be able to use the NAUPA format for all your reports.

Electronic Reports.
You may file electronically using the software below or others that meet these requirements:

  • must be in the NAUPA II format
  • must be electronic for businesses submitting more than 10 properties
  • must be separate files for tangible and intangible property

If you send an electronic copy, the signature requirement is waived.

Written Reports / Forms
** The following forms are available to businesses reporting nine or fewer properties. (The Unclaimed Property Program requests holders reporting 10 or more properties to report electronically.)

You may type directly into the reporting forms and save, print, and e-mail your information to us! Please note, the Treasury Department cannot accept electronic versions of the Holder Request Reimbursement
(AP-5) due to the notary requirement. However, you can still type into the AP-5, save and print the form, and then mail it to us.

The forms are furnished in an Adobe (.pdf) format. If you have Adobe Reader installed as a plug-in to your browser, you should be able to type into, save, and print these forms from your computer. Download Adobe Acrobat. *You will be redirected from Treasury's Web site to an independent site.

Negative Reporting (No Unclaimed Property to Report)
If you wish to manually file a negative report indicating that you have no unclaimed property, you may type directly into the form, print, and mail it or send an electronic copy via e-mail to unclholders@patreasury.gov.

If you wish to electronically file a negative report, your report will be automatically uploaded into the unclaimed property database and no hard copy submission is necessary. By filing a negative report, when your company or organization does not have any reportable unclaimed property, will help maintain an “active” reporting status. Online Negative Reporting

Summary Sheet of Reported Items (AP-3)
This one-page form to summarize the property you are reporting and the amounts you are remitting should be filed with the Pennsylvania Treasury Department, along with Forms AP-1 and AP-2, no later than April 15 of each year. Download form...

This three-page form (plus a one-page instruction sheet) is for completion by holders seeking reimbursement from the Pennsylvania Treasury Department for funds turned over to Treasury, but for which the rightful owner (or owner's representative) has since submitted a claim. You may type into the form, print, and mail it ONLY. Treasury will not accept this form electronically. The notarized statement on page three must be completed before Treasury will process the request for reimbursement and make payment.

This three-page form (plus a one-page instruction sheet) is for completion by holders seeking reimbursement from the Pennsylvania Treasury Department for funds turned over to Treasury, but for which the property was remitted in error. Instructions | Download form...
